Share your experiences of NHS Health Checks in Norfolk

We are working with Norfolk County Council (NCC) to understand more about people’s experiences and uptake of NHS Health Checks in Norfolk.

An NHS Health Check will check for certain health problems or early signs of some conditions. If you are aged between 40-74 years old, you may be eligible for an NHS Health Check and we want to hear from you.

Your GP should contact you to book your NHS Health Check. You can either go to your practice for the check or book online. Whether you have had multiple NHS Health Checks or have never had one before, your views are important to us. Hearing directly from service users will allow NCC to gain a better understanding of the public’s uptake and experiences of NHS Health Checks as well as opportunities for learning.

The survey should take around 10 minutes to complete. All responses are anonymous and will be fed back to NCC. To take part, please visit the smartsurvey website.

If you would prefer to complete the survey by phone, you can call Healthwatch Norfolk on 01953 856029 and we will arrange a time to ring you back to complete the survey.

More information about the survey can be found on the Healthwatch Norfolk website.
